Telehealth Informed Consent

What is telehealth?

Telehealth uses secure video and related technology to provide psychiatric care remotely. Your provider will determine whether telehealth is appropriate for your needs; some situations may require in-person care or referral.

Provider identity and licensure

Your care is provided by a licensed Psychiatric Mental Health Nurse Practitioner. Your provider will identify themselves and their credentials and practices only in states where they are licensed. We currently serve patients located in Washington. You must be physically located in a state where your provider is licensed at the time of your visit, and we may verify your identity and location.

Privacy and security

We use HIPAA-compliant, encrypted technology for telehealth visits and protect your information as described in our Notice of Privacy Practices. As with any technology, some privacy and security risks remain. For your privacy, attend visits from a private location.

Benefits and limitations

  • Benefits: convenient access to care without travel, often with greater scheduling flexibility.
  • Limitations: technology can fail or be delayed; a provider cannot perform a physical exam; and telehealth may not be appropriate for all conditions.

Emergencies

Telehealth is not for emergencies. If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency room. If you are in crisis, call or text 988 (Suicide & Crisis Lifeline). At intake we will confirm your location and an emergency contact so we can direct help if needed.

Your consent

By participating in telehealth visits, you acknowledge that you understand the above. You may withhold or withdraw consent at any time, and you may ask to be seen in person or referred elsewhere.
